• Please download and fill in Medical History.
• Bring in any available Pano(ramic) and/or Ceph(alometric) X-rays (best: digital copies), pictures, molds of your teeth.
• IMPORTANT: Tricare OCONUS (active duty) beneficiaries - please bring a Non-Availability and Referral Form (NARF) – you can request it at Dental Clinic, U.S. Army Garrison Stuttgart.
